What do you think about this?

What is this essay about? It seems a contrivance for a heavy handed morality tale based on an incomprehensible situation. Also, which country is the story based in? I don't know of a civilized country where a doctor would legally be allowed to refuse treatment based on ability to pay. There is a requirement for suspension of known fact in order to relate to your story, which would usually result in a detachment from the message you are trying to convey. Also, if the doctor was so heartless that he would not even consider seeing the patient without being paid, why would he be emotionally affected by the patient's death?
